At Care Unity, we offer a range of Nurse Support Packages designed to meet the diverse needs of our clients. Each package is tailored to provide the right level of care and support, whether you're looking for a simple annual assessment or more frequent, comprehensive care. Below, you'll find a breakdown of what each package includes:

Basic Support Package

Schedule: Annual Visit

Inclusions:

  • Comprehensive health assessment performed annually by a registered nurse.

  • Review of medical history, medications, and overall health status.

  • Basic physical examination, including vital signs check (blood pressure, heart rate, etc.).

  • Health education and recommendations for maintaining well-being.

  • A detailed report provided to the client and their healthcare provider.

  • Access to Care Portal: Includes access to a care portal with real-time updates and communication between family members and the care team.

 

 

Standard Support Package

Schedule: Quarterly Visits (every 3 months)

Inclusions:

  • Everything in the "Basic" tier.

  • Quarterly follow-up visits for ongoing health monitoring.

  • Coordination of care with other healthcare providers (e.g., GP, specialists).

  • Medication management support, including prescription reviews and reminders.

  • Personalized health care plan development, including diet and exercise recommendations.

  • Access to Care Portal: Includes real-time updates and direct communication with the care team.

 

 

 

Caregiver

Premium Support Package

Schedule: Monthly Visits

Inclusions:

  • Everything in the "Standard" tier.

  • Monthly nursing visits for continuous care and health management.

  • Wound care management, including dressing changes as needed.

  • Continence management support, including assessments and product recommendations.

  • Support with chronic disease management (e.g., diabetes, heart disease).

  • Regular communication with family members or caregivers to keep them informed.

  • Access to Care Portal: Full access with real-time updates, care plan adjustments, and family engagement.

Platinum Support Package

Schedule: Bi-Weekly Visits (every 2 weeks)

Inclusions:

  • Everything in the "Premium" tier.

  • Bi-weekly nursing visits for intensive health monitoring and care.

  • Palliative care support for clients with life-limiting conditions.

  • Post-operative care, including wound management, medication, and rehabilitation support.

  • Emergency care coordination and 24/7 on-call nursing support.

  • Assistance with navigating government and private funding options.
